Case Manager

SCOPE OF WORK

The duties listed below are intended only as illustrations of the various types of work that may be performed. The omission of specific job duties does not exclude them from the position.

1. Conduct client assessments and evaluations and make recommendations.

2. Coordinate client services.

3. Develop and maintain referral sources.

4. Make referrals to appropriate resource agencies.

5. Review files and records.

6. Review client files for accreditation and licensure compliance.

7. Work with all assigned clients to maintain stability for treatment.

8. Work with clients in treatment or waiting treatment to effectively manage medical, financial, legal or other issues that are barriers to successful completion of the treatment episode.

9. Participate in clinical staffings and team meetings.

10. Monitor random urine drug screens.

11. Prepare and maintain clinical and program records.

12. Testify in court proceedings.

13. Assist in the training and education of other staff members, interns and students.

STANDARDS OF PERFORMANCE
1. Maintain client rights as defined by DCF.
2. Maintain the highest standards in ethical conduct and confidentiality.
3. Maintain clinical records in accordance with Agency standards.
4. Follow all established Agency policies and procedures.
5. Model ideals of integrity, loyalty, and teamwork.
6. Produce a minimum of 1200 client contact hours (units of service) annually.
7. Sustain a positive, caring, supportive, recovery-oriented environment

EDUCATION REQUIREMENTS

• Vocational Training • Masters Degree

CERTIFICATION/LICENSE REQUIREMENTS
• CAP, CAPP or CCJAP

RELATED EXPERIENCE
One to Two years 

KNOWLEDGE, SKILLS & ABILITIES


Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to deal with individuals under stressful situations. Knowledge of crisis management techniques. Knowledge of infection control procedures. Knowledge of 12-step program, behavior modification, case management and addiction.
