Case Manager (CHS)

PRIMARY FUNCTION

Provide case management for clients in the homeless program who may be diagnosed with chemical

dependency or co-occurring chemical dependency and mental health disorders by performing the

following duties.

SCOPE OF WORK

The duties listed below are intended only as illustrations of the various types of work that may be

performed. The omission of specific statements of duties does not exclude them from the position.

1. Administer a standardized housing intake.

2. Develop a housing plan with each client and coordinate with appropriate programs depending on

housing placement

3. Develop transition and discharge plans with clients that include provisions for housing,

employment and behavioral health disorders.

4. Provide 1:1 CM activities to assist clients in meeting service goals, by meeting with weekly and/or

biweekly.

5. Facilitate life skills group with fidelity to evidence-based practices.

6. Prepare and maintain clinical records within required time frame.

7. Maintains meaningful involvement with consumers, advocates, and community partners

8. Submit reports as required.

9. Participate in client staffing and team meetings.

10. Make referrals to appropriate resource agencies and document outcomes of referrals.

11. Knowledge of program policies and procedures;

12. Provide follow-up and aftercare services as needed;

EDUCATION REQUIREMENTS

Undergraduate Degree in a related field 


RELATED EXPERIENCE

¨Two Years 

KNOWLEDGE, SKILLS & ABILITIES

Ability to read and interpret documents such as safety rules, operating instructions, and procedure

manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems

and deal with a variety of concrete variables in situations where only limited standardization exists. Ability

to interpret a variety of instructions furnished in written, oral, or schedule form. Knowledge of homeless

services preferred, 12-step program, behavior modification, case management and addiction. Knowledge

of Rental Assistance programs, Rapid Rehousing Funding, Permanent Supportive Housing.
