PRIMARY FUNCTION
Provide assessments, evaluations, and case management services for patients seeking treatment for Behavioral Health Disorders.
SCOPE OF WORK
The duties listed below are intended only as illustrations of the various types of work that may be performed. The omission of specific job duties does not exclude them from the position.
1. Conduct patient assessments and evaluations to make recommendations for treatment plans.
2. Coordinate internal and external client services.
3. Assist patients in effectively managing medical, financial, legal or other issues that present barriers to successful completion of treatment.
4. Develop, maintain, and make referrals to appropriate resource agencies, as needed.
5. Create and implement patient transition and/or discharge plans.
6. Participate in clinical staffing and team meetings.
7. Maintain required documentation in accordance with Agency standards.
8. Complete and keep up-to-date all required Agency trainings and development initiatives.

RELATED EXPERIENCE
- Two to Four Years
KNOWLEDGE, SKILLS & ABILITIES
Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to manage high-stress situations with individuals displaying behavioral outbursts. Knowledge of crisis management techniques. Knowledge of infection control procedures. Knowledge of 12-step program/addiction, behavior modification, case management and mental health disorders.
