Pregnancy Specialist

Primary function

  • Provide case management services and connection to community resources for pregnant and post-partum women.

Scope of work

  • Maintain caseload as assigned by Counselor Supervisor.
  • Develop, maintain, and document case management plans via the EHR for assigned patients.
  • Engage and link patients to appropriate community resources and document outcomes in EHR.
  • Act as a liaison between community partners, resources and COVE.
  • Maintain a current log of appropriate referral sources for pregnant and post-partum women.
  • Attend weekly pregnancy treatment team meetings and record notes.
  • Act as assigned COVE representative for community meetings, as needed.
  • Complete monthly reporting for various funder requirements.
  • Maintain documentation in accordance with regulatory standards.
  • Participate in clinical staffing and other COVE treatment team meetings, as indicated.
  • Provide and assist in coordinating patient transportation to community appointments and events, as needed.
  • Complete screenings for child development and make referrals as needed.
  • Conduct pre and postpartum depression screenings with participants served.
  • Attend COVE and external trainings to increase knowledge to best assist participants served.

Compensation: $16.09
