Patient Engagement Supervisor

PRIMARY FUNCTION

Supervise day to day operations of Community Outreach team and implement Agency standards. Provide supervision and training. Directs personnel engaged in providing outreach, HIV\HEP C testing and education, health services linkage, general data collection and planning /coordinating community events by performing the following duties.

SCOPE OF WORK

1. Provide direct supervision of all Community Outreach employees and intervention specialists.

2. Direct daily operations, respond to inquiries, and serve as the point of contact to resolve conflicts.

3. Plan and conduct staff development activities as required.

4. Provide staff orientation, performance evaluations, role modeling, scheduling and training.

5. Monitor employee morale and recommend ways to improve retention and job satisfaction.

6. Provide ongoing training and guidance to staff on HIV evidence-based practices as well as motivational interviewing strategies to engage prospective patients into treatment at Cove.

7. Identify locations and develop community partnerships to ensure that Outreach occurs in relevant locations across Tampa Bay and is provided to high-risk individuals in the community.

8. Recruit, educate and test high risk individuals in the community through outreach.

9. Coordinate HIV/HEP C testing services and behavioral health screening as needed.

10. Coordinate HIV and Behavioral Health and education presentations.

11. Consult with Cove's Director of Nursing and Medical Director on emerging community public health trends and responses needed from community as needed.

12. Ensure that contracted outcomes and deliverables are met and develop and submit required reports to Director and funders weekly, monthly, annually and as needed.

13. Assist Director of Nursing with tasks related to managing department as needed.

14. Cultivate, maintain and enhance relationships with major referral sources to facilitate linkages.

15. Ensure accurate and timely submission of client data.
