Case Manager (FLITE/FITT)

PRIMARY FUNCTION

Provide in-home assessments and case management services for clients at risk for or diagnosed with

chemical dependency or co-occurring chemical dependency and mental health disorders that are also involved with the child welfare system. Significant local travel required with no out of state travel.

SCOPE OF WORK

The duties listed below are intended only as illustrations of the various types of work that may be

performed. The omission of specific job duties does not exclude them from the position.

1. Conduct client assessments and evaluations and make recommendations.

2. Coordinate client services.

3. Coordination with referral sources.

4. Make referrals to appropriate resource agencies.

5. Review client files for accreditation and licensure compliance.

6. Work with all assigned clients to maintain stability for treatment.

7. Work with clients in treatment or waiting treatment to effectively manage medical, financial, legal

or other issues that are barriers to successful completion of the treatment episode.

8. Participate in clinical staffings, court hearings and team meetings.

9. Monitor random urine drug screens.

10. Prepare and maintain clinical and program records.

EDUCATION REQUIREMENTS

RELATED EXPERIENCE

KNOWLEDGE, SKILLS & ABILITIES

Ability to read and interpret documents such as safety rules, operating instructions, and procedure

manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems

and deal with a variety of concrete variables in situations where only limited standardization exists. Ability

to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to deal with

individuals under stressful situations. Knowledge of crisis management techniques. Knowledge of

infection control procedures. Knowledge of 12-step program, behavior modification, case management

and addiction.

PHYSICAL REQUIREMENTS
