Care Coordinator

Scope of work:

The duties listed below are intended only as illustrations of the various types of work that may be performed. The omission of specific statements of duties does not exclude them from the position.

  • Perform initial consultation to determine client-specific needs and goals.
  • Educate clients on Agency roles and treatment options available.
  • Develop care plans in coordination with Agency providers and treatment options.
  • Coordinate internal and external substance use, mental health, and/or physical health services afforded to each client in support of the individualized care plan.
  • Consult with client care team to evaluate assigned interventions/treatment to ensure congruity with continued care plan.
  • Continually review care plan with client to ensure appropriate treatment and education as client progresses.
  • Initiate care plan updates and additional interventions when appropriate for client progress.
  • Provide crises intervention, as needed.
  • Submit reports and complete documentation as required.

Education:

  • Undergraduate Degree and 2 to 4 years of related experience (required).
  • Must be able to pass background and drug screen prior to start date.

Job Type: Full-time (Non - Exempt).

Compensation: $16.83 USD per hour.
